2012 Music Booster Summer Camp Scholarship Application
The 2012 Music Booster Summer Camp Scholarship Application is now available.
Please submit this application to the Booster Box in an envelope with your name and “Summer Camp Scholarship Application” on the front. Your family must be a member of Music Boosters and you must have participated in Booster Fundraisers in order to qualify for a scholarship. No applications will be accepted after the May 1st, 2012 deadline. Recipients will be announced at the Awards Banquet.
Music Booster's Membership Form
Please print and complete the Music Booster's Membership Form, include your $10.00 fee and mail to:
Carl Sandburg High School – Music Boosters, 133rd & LaGrange Road, Orland Park, IL 60462. Deadline for turning in Music Booster Membership fee is October 31st.
*You must be a member of Music Boosters for your student to be eligible for scholarships!*
DVD Order Form
If you would like to order a DVD of the concert performances, please print and
complete the new Music Booster's DVD Order Form.
DVD’s are $15.00 each. If you would like to have the
DVD mailed to Student or Parent - please add $2.00 to your order Total. Be sure to identify the Date of the Concert on the form and check the appropriate boxes. Please make checks
payable to "Carl Sandburg Music Boosters" with the student's name in the memo section. Place the completed form, along with a check, in the Music Booster's Mailbox. Fundraising Guidelines and Options
Please consider your desired option in advance, however, if you are undecided or confused we can help you at schedule pick-up. The Golden Opportunities Books will be available at each of the pick-up dates. Please review the Fundraising Guidelines and Options.
